Preparing for your Visit
Please review this list of items to ensure that you have everything needed for your appointment.

 • Picture ID
• Health Insurance Card
• Co-pay, if Required
• Referral, if Required
• Completed Forms (All Sections)
• X-rays, CT Scans, and Other Relevant Tests - Please bring
  the films/disc and the report
• List of All Current Medications, including Over-the-Counter,
  Vitamins, and Supplements
• List of Questions and/or Concerns
• DVD for Allergy Testing for Children


Guidelines for Improved Service
• Arrive Early - Patients who arrive late may need
  to be rescheduled.
• Minor patients (under age 18) need to be accompanied
  by a parent or guardian.
• If you are bringing a child, please remember to pack toys,
  books, or other items to "entertain" and distract your child.
• Review the above instructions and be sure that you have
  brought all applicable information/paperwork.
• Please be patient - We strive to be prompt with all
  of our appointments, but occasionally an emergency
  may take precedence.
