Contact Information For surgical scheduling-related items, contact the Surgical Coordinator at extension 108. Insurance Cards A copy of your insurance card will be made and kept with your medical records. X-Rays, CAT Scans & Tests Results Bring any CAT scans, x-rays or test results relating to your ENT or hearing problem with you to your appointment. This helps avoid delays in your care & treatment. Save Time In Our Office Completing Forms The following forms can be downloaded to your computer and printed at home. By filling out your forms ahead of time, you will save significant time at your visit. If you have any questions when filling out these forms, please do the best you can; our staff will assist you with your questions on the day of your visit. New Patient Registration Form Communication of Health Information Form Release of Records Authorization Privacy Policy and Patient Consent Form Sinus Patients - Nasal Sinus Questionnaire Dizziness & Balance Questionnaire Hearing Questionnaire Snoring and Sleep Apnea Questionnaire Tinnitus Assessment Cancellations If you are unable to keep an appointment, we ask that you kindly provide us with at least 48 hours notice. This courtesy on your part will make it possible to give your appointment to another patient. Fees and Insurance Patients are responsible for providing our office staff with current and correct insurance information so that reimbursement claims can be processed quickly and accurately. We request that you contact your primary care physician for any referral forms required by your HMO insurance for your appointment. It is your responsibility to see that the necessary referrals for services are available at the time the services are rendered. Co-payments & payments, which may be made by cash, check, Discover, American Express, Visa or MasterCard, are required at the time of service. Payments for services not covered by your insurance carrier are your responsibility.
